Meningkatkan Efektivitas Program Pengelolaan Penyakit Kronis Kardiovaskuler melalui Sinergi Layanan Kesehatan Primer di Kabupaten Cirebon

Cardiovascular diseases are among the major global health challenges, including in Indonesia, with the prevalence of hypertension and diabetes mellitus continuously increasing. The Chronic Disease Management Program (PROLANIS), organized by BPJS Kesehatan, aims to improve the quality of life for individuals with chronic diseases through a primary healthcare approach. This study aims to analyze the relationship between stakeholder synergy in primary healthcare services and the effectiveness of cardiovascular PROLANIS implementation in Cirebon Regency. Using a quantitative cross-sectional design, data were collected from 30 Primary Healthcare Center (Puskesmas) Heads through structured questionnaires and analyzed using Pearson correlation and simple linear regression. The results showed that most respondents rated the level of synergy as high, with an average score of 82.3. PROLANIS effectiveness, measured through participant routine visit rates (78.5%) and controlled blood pressure rates (72.2%), indicated relatively good achievements. Correlation analysis revealed a positive and significant relationship between stakeholder synergy and PROLANIS effectiveness (r = 0.615; p = 0.001), with synergy contributing 37.8% to the variation in program effectiveness. This study highlights the importance of strengthening synergy among local governments, healthcare facilities, BPJS Kesehatan, and communities to improve the effectiveness of chronic disease management programs. Consistent collaborative efforts, policy support, community empowerment, and community-based innovations are key strategies in addressing the challenges of cardiovascular disease management at the primary healthcare level.
